We have two forms available in Microsoft Word 6.0 format. These can be viewed in Microsoft Wordpad which comes with Windows, or Microsoft Word 6.0/97/2000/XP.
If you are a parent of a vision impaired child please fill in the Membership form. If you don't have a child with a vision impairment but you want to help out, please fill in the Friends/Volunteers form.
You can print out a form, fill it out and send it to Vizhelp c/o the Association for the Blind, PO BOX 101, Victoria Park, 6979.
Or, you can fill out a form on the computer and E-mail your completed form to <pam@vizhelp.org>.
Otherwise you can obtain a print copy by expressing your interest to <pam@vizhelp.org>.
